What to Expect During Your First Visit

During a typical initial visit to Studabaker Chiropractic, each patient can expect to fill out some new patient paperwork. There are pdf links on this page that will allow you to download and print off most of the required forms so they can be filled out ahead of time. These forms are always available in the office as well. We would simply ask that you arrive with enough time to fill out this paperwork before the scheduled appointment. There will be a brief consultation and overview of past medical history with Dr. Studabaker . A problem focused exam is then performed to identify a proper diagnosis. At that point, Dr. Studabaker will discuss your diagnosis and develop a personalized treatment plan.
